Routine Memberships

Included in Annual Membership:

2 routine visit recall exams per year (defined by duration of contract term)
2 cleanings per year (does not include cleaning in presence of periodontal disease)
One set of 4 bite-wing x-rays per year
One panoramic x-ray every 3 years
One Limited or Emergency exam per year
All Individual x-rays as needed for the Emergency Exams
20% discount on all other procedures performed in-office

Program Guidelines

Each additional family member**-$240 or $20/mo added with autopay
*Dual Membership is for Parent/Child or Spouse/Partner
**Dependents up to age 18 (or up to 23 if enrolled as a student full-time) living in the same household

Cannot be used with any other plan or insurance
No refunds of membership fee
